Adoption of a Minor §48

MINOR ADOPTIONS are handled by the Juvenile Court and ADULT ADOPTIONS are handled by the Probate Court.  

ALL DOCUMENTS SUBMITTED TO THE COURT SHOULD BE SUBMITTED IN DUPLICATE.  The Court will keep and file one set of copies.  If you would like the Juvenile Clerk of Court's office to make additional copies for you, the cost will be $1.25 per page.

 

Petition for Adoptive Placement: Petition to seek the court's approval for the placement of a child in a home for adoption.  A petition for termination of parental rights must accompany this petition.

Adoption Search Information: Wisconsin's Adoption Records Search Program works with adoptees, adults whose birth parents' rights were terminated, and birth parents to gain access to vital pieces of adoptees' social and medical histories.  Information can also be requested through the court using the Request to Disclose Adoption Court Records form.
