SEPARATE PETITIONS MUST BE FILED IF YOU ARE SEEKING MORE THAN ONE TYPE OF GUARDIANSHIP.
ALL DOCUMENTS SUBMITTED TO THE COURT SHOULD BE SUBMITTED IN DUPLICATE. The Court will keep and file one set of copies. If you would like the Juvenile Clerk of Court's office to make additional copies for you, the cost will be $1.25 per page.
SERVICE: The petitioner is responsible for service and/or notice. For additional information, please consult Wisconsin Statute 48.028(4)(a). Proof of service must be filed with the court no later than ninety-six (96) hours prior to the hearing.
