Parks and Land Use - Environmental Health  Well Water Sample Test Kits 

You can obtain a water sample kit for the tests you select from the options below:

Order by phone using a Visa/MasterCard or American Express by calling (262) 896-8300 (a $5 handling fee will be applied to the total cost of the analyses you select),

Pick up your kit(s) Monday through Friday, 8 a.m. to 4:30 p.m. at the same address.
Water Samples are accepted Monday - Thursday between 8-3:00pm. We do NOT accept samples on Fridays.
During a holiday week, please contact the office for lab hours.

PLU/Community Development HOME Consortium Housing Program Application, Lender Fees, and Title Company Payments

Currently these fees are collected by receipt of a physical check. 

1-The rehabilitation program requires a $75 application fee for each rehabilitation loan. With the paper-based checks, checks are held until income qualification is completed. If the application does not get approved the $75 application fee check is returned to the applicant. Only if the application is approved does the $75 get cashed.

2- In the Down Payment assistance program, the Lender pays a $200 fee, this fee is collected by physical check. An online system could potentially involve working with ACH transactions, e-check etc.

3-Loan closeout payments paid from a Title Company currently received by Title Company by physical check.

Parks and Land Use Order a Radon Kit

Test kits are generally available from hardware stores, supermarkets, and other retail outlets, and also Southeastern Wisconsin Radon Information Center. In Wisconsin you can call 888 LOW RADON.

Phone: Call us at 262-896-8300 to order the initial short-term kit for $10 (Visa • MasterCard • American Express • Discover only). Charge includes kit and processing at the manufacturer's laboratory. You must supply eight first-class postage stamps to send the kit to the lab. The lab will send the test results directly to you.

In-Person: Purchase a short-term or long-term kit at Waukesha County Environmental Health. Find us in room AC 260 of the Waukesha County Courthouse at 515 W. Moreland Boulevard in Waukesha (directions)

Medical Examiner Cremation Permit The fee for 2025 (effective 1/1/2025) is $302

The Medical Examiner's office accepts payments in the form of check or cash.

For your convenience, the Cremation Permit Request form is a fillable electronic form Cremation Form. Request forms must be submitted to our office any time a cremation permit is needed.

Medical Examiner Death Certificate The fee for 2025 (effective 1/1/2025) is $97

The Medical Examiner's office accepts payments in the form of check or cash.

Our office will start all death records in SVRIS (state vital records system) when we are certifying. Funeral homes will be granted access to the respective death record after we speak with the family and confirm funeral home selection.

In order for remains to be disinterred from our county, our office must issue a Disinterment Release. Before issuing the form, we must receive the original, notarized Request for Disinterment Permit form and payment in full.

A dropbox is located on the east side of the Waukesha County Administration Building, across from Door 35, facing Pewaukee Road. Any tax payments deposited in the dropbox by 7:30 a.m. will be dated for the previous day.

Circuit Court Obtain Copies of Documents from a Court File

The Civil, Criminal/Traffic, Family, and Juvenile divisions must charge a statutory fee of $1.25 per page for copies of official court documents. Copies can be certified (stamped as official copies) for an additional $5.00 charge for each document.

The Register in Probate must charge a statutory fee of $1.00 per page for copies of official court documents. Copies can be certified (stamped as official copies) for an additional $3.00 charge for each document.

In Person:
Go to the Circuit Court Division where the case is filed. You will need to know the case number (which may be located on WCCA) or the last name, first name, and date of birth of one of the parties for certain cases. If you do not have this information and you are unable to find the individual or record using a public access computer, you may complete the Record Search form and pay the $5.00 search fee to receive assistance with the record search (this fee is $4.00 in Probate Court). If you would like copies of any pages, DO NOT REMOVE THE PAGES, but place a paperclip on each page of which you want a copy.

By Mail:
Send a written request to the Circuit Court Division where the case is filed. You will need to know the case number or last name and date of birth of one of the parties for certain cases, the specific document or information you need, a self addressed stamped envelope, and a phone number at where the clerk may reach you. If you do not have this information and you are unable to find the information using a public access computer, you may complete the Record Search form and pay the $5.00 search fee to receive assistance with the record search (this fee is $4.00 in Probate Court). Note: Payment in full will be required before your request will be processed.

By Phone:
Call or Fax the Circuit Court Division where the case is filed. You will need to know the case number, or last name, first name, date of birth of one of the parties for certain cases, and the specific document or information you need. The clerk will make arrangements with you for payment and pick-up/delivery, by mail or in person. Note: Payment in full will be required before your request will be processed.
