Health and Human Services Centralized Records
Our Centralized Records Team is responsible for the management and release of records within Health and Human Services. We work to ensure your client records and health information remain secure and are made available to you in a timely manner. We also respond to public records requests for Health and Human Services.
Our hours of operation are 8:00 a.m., through 4:30 p.m., Monday through Friday, excluding holidays. Our team is available to assist if you have any questions regarding records requests.
How to Request Your Client Records / Health Information
We will require you to fill out and sign an Authorization for Use & Disclosure of Health or Confidential Information form when:
- You request copies of your personal client records and health information.
- You want to send copies of Health Information to another health care provider for treatment purposes.
- You want to send your Health/Client Information to third-parties.

If you request that your mental health, substance use disorder, HIV testing, or other human/social services records be released, you must check those specific boxes on the form.
The authorization form is not valid unless filled out completely.
The completed forms can be mailed, faxed, emailed, or delivered to the following locations:

Picking Up Records
To ensure the security of your client records and health information, a photo ID is required when picking up records at our locations above. You may designate someone on the authorization to pick up records on your behalf, but that individual must provide a photo ID as well.
If you do not have a photo ID, please let us know ahead of time and we will work with you to ensure we can verify your identity.
Public Records Requests
Requests to inspect or receive copies of public records for health and human services may be submitted at our Human Services Center above.
