There is an option to fill out a paper application to turn in at the local agency or to interview in person. There is even an option to start a short-form application (via phone, online, or in person) to start the process and finish later. Lobby services are open from Monday - Friday, 8:00 AM to 4:15 PM. - Paper Application: Prefer a physical form? Request one be mailed to you by calling the same number. Alternatively, download forms for various programs right here: FoodShare, BadgerCare Plus, EBD Medicaid, and Wisconsin Shares Childcare Subsidy.
- In-Person Help: Visit Waukesha County Health and Human Services Center where customers can get started on applications or staff can assist. Participants can drop off case documentation to be sent to the Central Documentation Processing Center in order to be assigned to a caseworker for processing.
- Using Technology: For document submissions, use the MyAccess smartphone app available on Android and Apple devices. Simply take photos of documents and upload them straight from participant’s phone. Participants can also submit documentation using the ACCESS.wi.gov website.
- Replacing FoodShare Cards: Receive your permanent FoodShare card by mail via QUEST. If your card is lost, stolen, or damaged, call QUEST at 1-877-415-5164 for a replacement.
- Document Submissions: Drop your documents in the box located at the HHS main entrance or at Economic Support Services on the lower level of the HHS building. Participants can also submit documentation via mail or fax or using the ACCESS.wi.gov website or using the smartphone application “MyACCESS” which is available for no cost at the Google and Apple app stores.
- Address for mail should go to CDPU, PO Box 5234, Janesville, WI 53547 (remember to add your case number to the documentation so it is attached to the appropriate case).
- Fax to the Central Document Processing Center at 1-855-293-1822 (remember to add your case number to the documentation so it is attached to the appropriate case).
