 Proposed Dispositional Order Appointing Guardian – Form No.:  IW-1606

 Notice of Hearing – Form No.: IW-1724 

GENERAL INFORMATION

After you have obtained Service on all interested parties, you must complete and file with the Court:

 Affidavit of Service (Chapter 48 and 938) - Form No.: JD-1825

Per Wisconsin Statutes, a guardian ad litem will be appointed for the minor at the time of case filing.  

Notice must be given to all interested parties.
