Children’s Long Term Support Waiver Program (CLTS)
The Children’s Long Term Support Waiver Program (CLTS) helps children and youth under the age of 22, with limitations in daily activities, live at home and take part in their communities. These limitations are a result of an intellectual and/or developmental, physical, or mental health disability.
The goal of the program is to provide supports and services to help children and youth stay in their homes. The program uses a team approach to decision-making called Deciding Together to create an Individual Service Plan. The program utilizes Medicaid funding to pay for services. Eligibility
All children or young adults must meet the basic requirements listed below for CLTS Waiver Program eligibility. To qualify for certain types of funding, there may be additional requirements to participate in the CLTS Waiver Program, your child must meet the following criteria:

Be younger than age 22.

Be eligible for Wisconsin Medicaid.

Live at home, in foster care, or in another approved setting.

Need a level of care that people get at:

A hospital.
-
A nursing home.
-
An institution for people with developmental disabilities.

Be able to get safe, required care at home or in the community.
The Children’s Community Options Program (CCOP) provides a coordinated approach to support families who have a child or youth with a disability reach their goals. The program services help your child or youth become more independent and achieve identified outcomes. CCOP pays for many supports and services that you can’t get through other insurance like Medicaid or the CLTS Program.

The Waukesha County Department of Health and Human Services (HHS) completes an annual Consumer Quality Assurance Survey within the CLTS Program. The Consumer Quality Assurance Survey is part of an ongoing commitment to continuous improvement and quality service. Our goal in the CLTS Program is to provide the best possible program experience for families.
