Court Appointed Attorney Information
For all attorneys who have been appointed or wish to be on the attorney appointment database as a Guardian Ad Litem or Advocate Counsel, please complete the Court Appointed Counsel Agreement and Compensation Agreement and Appointed Attorney Profile Information forms. In addition, a copy of your Certificate of Insurance must be submitted. Return forms via email to the Programs and Projects Analyst. All agreements will be reviewed and approved by the Clerk of Courts and Presiding Judge.
An Affidavit of Services Rendered must accompany each invoice submission.
State Bar GAL Practice Guidelines should be reviewed by all Guardian Ad Litems practicing in Waukesha County.
